Here are the 3 easy steps I recommend you to follow in order to overcome your approach anxiety in just a couple of days:
1. Observe
Try going out for at least one day and observe women. Bars, clubs, parties, shopping or, simply, on the street... it doesn't matter. Wherever you go, just observe them. While you're watching, think in terms of opportunity: what would your opener be for that particular situation. The idea is to push yourself well passed the point where you feel like you could use that opener and actually approach the woman in that particular situation.

2. Say "Hello"
In the following two days, I want you to say "hello" to every woman you pass by on the street and continue walking. If you feel embarrassed to do this, try wearing sunglasses so they won't be able to see you very well and you won't even have to look at them... They won't know any difference. So every time you pass by a women say "hello", or "hi, how are you doing?" if you can, then just keep walking. You'll be surprised to get some responses too... but you will just keep walking. You'll get to the point where you just can't wait for the next day when you can actually DO something.
3. Proceed
Try to remember some of the openers you've been creating in the first day, say "hello" to women and try an opener every time you feel like doing it. Try to start a conversation with as many as you can. By this time, you'll realize that you've already overcame your anxiety to approach women.
